Always thought they were due to ischaemia from elevation but I have been corrected by a student. Those relieved by stretching, not dependence, are neurological etiologically.

Nocturnal leg cramps mostly respond to magnesium 3-400mg/day as it is nature's calcium channel blocker and is essential for parathyroid hormone which improves the absorption and retention of calcium. Most practitioners use a 2:1 ratio of Calcium: magnesium in the long term, eg 500mg calcium (citrate is much better absorbed than carbonate) with 250mg magnesium - many good supplements have this sort of level. Topical magnesium (magnesium gel or magnesium oil) has worked well for my patients with idiopathic nocturnal cramps.
Oral magnesium, due to its limited absorption, has not been as effective as the topical forms in my experience.

A surprisingly common complaint in the office, the nocturnal leg cramp, with limited useful suggestions for effective remedies. Will try the Magnesium/Calcium suggestion: hey, what harm can it do. Spinal and pseudo-spinal stenosis deserves a closer look too.

Alcohol consumption is associated with nocturnal leg cramps
Association between alcohol consumption and nocturnal leg cramps

    AMERICAN ACADEMY OF FAMILY PHYSICIANS

New research finds that, among patients over 60 years old, there is a strong association between consumption of alcoholic beverages and nocturnal leg cramps. In a case control study in France, 140 general practice patients with and without leg cramps were administered a food frequency questionnaire. Researchers found an association between global consumption of alcoholic beverages and nocturnal leg cramps. Patients drinking alcohol at least once a week had an odds ratio of 6.5 of suffering from nocturnal leg cramps. There was no linear relationship between amount of alcohol consumed and odds of leg cramps. In light of the negative effect that nocturnal leg cramps have on patients' quality of sleep and quality of life, the authors call for additional research to evaluate the existence of a causal link and to determine the pathophysiology of leg cramps and alcohol's impact on them.
